Join Us for Mini Mentor Cup

I understand the nature of First Tee – Greater Charleston Program at Charleston Fun Park as well as the Participant’s experience and capabilities and believe the Participant and Adult Mentor to be qualified to participate in the Program. I hereby release, discharge, covenant not to sue, and AGREE TO INDEMNIFY AND HOLD HARMLESS First Tee – Greater Charleston and its directors, officers, members, employees, contractors, volunteers and agents from and against any and all expenses, damages, claims, suits, actions, judgments, and costs, including reasonable attorneys’ fees, medical bills, arising out of or in any way connected with the Participant and Adult Mentor’s participation in the Program or the negligence of the Indemnities, including negligent rescue operations for the Participant. 

In the event that I cannot be reached in an emergency, I agree to accept any and all determinations of need for medical assistance and/or administration of medical attention deemed necessary by First Tee – Greater Charleston representatives. I hereby give permission to the medical personnel selected by First Tee – Greater Charleston representatives to secure any and all medical, hospitalization, dental, and/or surgical treatment. In the event that such medical attention is needed from a healthcare provider, all costs shall be the responsibility of the parent or guardian or Adult Mentor.

I understand that any golf equipment received for use is the property of Charleston Fun Park and must be returned at the completion of the event.

I hereby give First Tee – Greater Charleston, Charleston Fun Park, Headquarters Office and participating agencies permission to use film, video tape and/or photographs of the above mentioned minor, Adult Mentor, and any other spectators attending for lawful promotional or informational purposes.
